We were lucky to catch up with Abhi Kapuria recently and have shared our conversation below.
Abhi, thanks for taking the time to share your stories with us today If you could go back in time do you wish you had started your business sooner or later
I wish I did this sooner but don’t know if it would have been possible. I was a busy Neurologist, seeing patients, juggling family and life and moving cities for my wife’s work. I thought doing some legal work on the side would help reduce my clinical demand. I found that I was really good at it and that I could create systems for others to join us which snowballed. Unfortunately starting earlier just wasn’t realistic with the number of years of training it takes to be a physician but starting later would have been a huge mistake. Starting later would have probably burnt me totally out of clinical medicine and demotivated me from making the leap to entrepreneurship as my family grew and my responsibilities at work compounded. I wish I had better mentorship about the plethora of opportunities there are beyond strictly employed work.

Great, appreciate you sharing that with us. Before we ask you to share more of your insights, can you take a moment to introduce yourself and how you got to where you are today to our readers.
Medical-legal work was not something I set out to do. I went into neurology because I was drawn to the complexity of the brain and the stakes involved. Stroke, traumatic brain injury, seizure disorders, these are conditions where details matter and outcomes matter. The science keeps evolving, and you have to stay sharp.
My entry into expert work actually came through my co-resident, who is now my business partner. He had started doing consulting work with attorneys and showed me what it really looked like. We saw that there was room to do it better, to build something thoughtful and disciplined rather than transactional. That is how Neuro Legal Consulting started.
Through that practice, I review medical records, provide objective opinions, write reports, and testify when needed. I work with both plaintiff and defense attorneys. The core of the job is translating complicated neurological medicine into language that makes sense in a courtroom without oversimplifying it.
I think what separates me is that I am still in active practice. I see patients. I manage real strokes and seizures. At the same time, I have experience in depositions and trial testimony, so I understand the legal environment. I take cases I believe in and I write opinions I am comfortable defending.
Now we are expanding that model into a broader physician network called Apex Med Law. The goal is to bring the same level of preparation and credibility across multiple specialties. At the end of the day, this work comes down to trust. My role is to give an honest assessment of the medicine and explain it clearly, especially when the pressure is on.

How’d you build such a strong reputation within your market?
Early on, we were intentional about building a team with real breadth and depth in neurology. We brought on physicians who are actively practicing across a wide range of subspecialties, from neuroimmunology and peripheral nerve disorders to vascular neurology and spine. The goal was simple: for any neurological issue that comes through the door, the right specialist is reviewing it.
That foundation matters. When the initial opinion is coming from someone who truly lives in that specific area of medicine, it sets the tone for the entire case. It gives attorneys confidence early, and it strengthens everything that follows.
Because of that approach, we have never had an expert disqualified, and we are able to bring a high level of clinical depth and credibility to every case we take on.

What’s worked well for you in terms of a source for new clients?
We maintain a presence on several professional registries, and our website also generates consistent organic traffic. Over time, we evaluated which registries were actually producing meaningful inquiries and eliminated the ones that were not delivering results. Instead, we focused our resources on the platforms that were performing well, investing in higher visibility and featured placements for our physicians where it made strategic sense.
