Prisiliano & Monica, looking forward to hearing all of your stories today. Setting up an independent practice is a daunting endeavor. Can you talk to us about what it was like for you – what were some of the main steps, challenges, etc.

Salveo Direct Care was born from a desire to take back medicine and deliver personalized health care in a revolutionary way. Four years ago, as co-owner and founder of a traditional primary care practice here in San Antonio, Dr. Prisiliano Salas grew weary of the traditional model of primary care – one dictated by insurance regulations, deductibles, and copays.

He felt deeply there was a better way to care for his patients and soon discovered Direct Primary Care. Direct Primary Care is a way of providing healthcare where patients pay a monthly fee to a medical practice who in turn provides them with all their primary care needs. There are no insurance bills, insurance co-pays or deductibles. The benefits of Direct Primary Care are allowing the doctor to provide more personalized and accessible care, encouraging patients to take better care of themselves knowing that there are less barriers to receiving the care they need and making healthcare more efficient for both the patient and the doctor.

Salveo Direct Care opened in August of 2018 and has been growing ever since. When the timing was right for their family, his wife and fellow Board Certified Family Medicine physician, Dr. Monica Salas joined the practice in April of 2021.

In the early days of the pandemic, Drs. Salas both felt another way to disrupt the medical industry was to build a practice focused on well care rather than sick care. Although they certainly care for acutely ill patients and those with chronic disease, the Salases appreciate that an ounce of prevention is worth a pound of cure. With this wellness mindset, Dr. Prisiliano and Dr. Monica both earned Board Certification in Lifestyle Medicine as well.

With any new business, particularly in the midst of a pandemic, there are always struggles. Fortunately, due to the practice structure and the nature of Direct Primary Care, the Salases have been able to face those challenges head-on and continue to thrive.

Given the direct agreement they have with patients, Salveo isn’t subject to insurance restrictions and is able to be more flexible with how services are provided to patients. They have utilized telemedicine and secure messaging from the beginning, which has allowed them to tailor patients’ care in a way that is not achievable in conventional practice.

Did you know that most Primary Care Providers have between 2,500-3,500 patients? In Salveo Direct Care, the patient panel is limited to much less. That means that your doctor spends more time with you when you need them. Plus, when they are not actively seeing a patient, they are able to help coordinate your care, research your treatments and create a tailored care plan with detailed lifestyle changes.

Drs. Salas are both Board Certified in Family Medicine and are Diplomates of the American Board of Lifestyle Medicine. They center their practice on evidence-based data and recommendations to treat and prevent acute and chronic issues, looking at the patient as an interconnected being. They strongly believe a pill is not always the solution and lifestyle changes should be part of patient care.

Patients can be a member from anywhere in San Antonio, Bexar County, and beyond with any of our plans and you still get the same care and cost savings. And both physicians are bilingual!

What’s been the most effective strategy for growing your clientele?

The majority of our patients have come to Salveo Direct Care via member referrals or word-of-mouth. Because the doctors provide each patient with a completely customized approach to their healthcare, patients feel seen and personally taken care of. There is no other healthcare experience that delivers the one-on-one focus that the Direct Primary Care model provides. As a result, patients are quick to recommend the practice to friends, family or other loved ones. And Salveo is happy to care for their needs as well!

How do you keep your team’s morale high?

Although owning and running a business while practicing medicine is certainly challenging, the rewards for Dr. Prisiliano and Dr. Monica are immense. By simplifying the medical practice structure, the Salases are able to operate with a small team and maintain a great deal of autonomy. Knowing that they are practicing medicine the way it was always intended to be practiced and restoring the doctor patient relationship makes each day of work rewarding.
