Alright – so today we’ve got the honor of introducing you to Elizabeth Komet PT, DPT, CSCS. We think you’ll enjoy our conversation, we’ve shared it below.
Elizabeth, looking forward to hearing all of your stories today. We’d love to start by getting your thoughts on what you are seeing as some the biggest trends emerging in your industry
One of the biggest trends I’ve seen in physical therapy—and in healthcare overall—is the shift away from traditional insurance-based models toward private, cash-based, and highly personalized treatment plans.
This trend is showing up in many forms. In my own practice, it looks like concierge-style care, where I travel to patient’s homes, work outside of insurance, and provide a hybrid of rehab, injury prevention, strength training, and Pilates. But such models are popping up in other healthcare professions as well from on-call concierge-family doctors, to boutique surgery offices, to home delivery IV drips, and much more.
There’s a wave of entrepreneurship sweeping healthcare as frustrations grow from both providers and patients with the current system. As a provider, it’s really hard to give quality care when you’re juggling multiple patients within the same session or being told what you can and can’t treat based on codes. And for patients, that often means rushed sessions, inconsistent care, and a lack of follow-through.
These alternative models allow for more time, greater continuity, and care that’s actually tailored to the individual—not just their diagnosis codes and insurance coverage. They’re also helping to reduce the burden on our overwhelmed healthcare system.
One of the biggest challenges with this private-pay trend is accessibility—right now, the highest quality, most personalized care is often only available to those who can afford to pay out of pocket. At the same time, these private healthcare businesses offer real value by helping to relieve pressure on an already overwhelmed system—where patients often wait weeks or months for care, or are only treated once they’re already injured or seriously ill. There’s enormous benefit in being able to address health concerns quickly and proactively. And when alternative models absorb some of the demand, it can also improve access and quality of care for those relying on traditional hospital systems and insurance-based clinics.
In my practice, I see myself as an extension of the broader healthcare system—lightening the load where I can. My goal is to keep people from needing to enter the system in the first place by focusing on proactive care: preventing injuries, surgeries, and chronic issues before they start.
As our society continues to face complex challenges in improving health outcomes, I think this trend creates space for more creativity, flexibility, and patient-centered solutions. Sometimes, the best way to support the system is to build something alongside it—something more accessible, more human, and ultimately more effective for everyone.
Awesome – so before we get into the rest of our questions, can you briefly introduce yourself to our readers.
Hi! I’m a Doctor of Physical Therapy (DPT), Certified Strength and Conditioning Specialist (CSCS), and movement educator with additional training in Pilates and yoga. I specialize in helping active individuals—including athletes, weekend warriors, and prenatal/postpartum women—move better, recover from injury, and build strength and confidence in their bodies.
My path into this field started with a deep love of movement and a curiosity about how the body works. I earned my Doctorate in Physical Therapy from the University of Colorado – Denver and have since built a career that blends evidence-based rehab with performance training and mindful movement.
I run a concierge physical therapy and fitness business in Los Angeles, offering personalized, one-on-one care in-home, in-gym, or virtually. My services bridge the gap between injury recovery and high-level performance. I help clients reduce pain, prevent injury, and return to the activities they love—whether that’s skiing, running, lifting, or simply keeping up with their kids.
What sets my work apart is the integration of biomechanics, strength and conditioning, and a truly holistic approach. I don’t just treat pain—I help people understand how they move, how they recover, and how lifestyle factors like sleep, nutrition, stress, and muscle mass impact their overall health. My goal is to build lasting resilience, not just quick fixes.
At the core of my brand is the belief that movement is powerful, healing, and essential for longevity. Whether you’re returning from injury, navigating postpartum changes, or working toward long-term performance goals, I’m here to support your whole journey—body, mind, and lifestyle.
Can you talk to us about how your side-hustle turned into something more.
Yes, my side hustle has absolutely become my full-time career—and it’s been one of the most rewarding journeys I’ve ever taken.
I started offering concierge physical therapy and fitness sessions on the side while working full-time in a physical therapy clinic. On weekends and after hours, I would jet off to see clients one-on-one in their homes or at the park—people who were looking for a more holistic, movement-centered approach. I combined physical therapy with strength training, mobility work, Pilates, and injury prevention—and brought it all directly to them, on their schedule.
To be honest, I was working A LOT—easily 50 to 60+ hours per week, not including commuting. But I’ve known since I was young that I wanted to run my own business, so even though it was exhausting, the hustle felt natural. I was energized by the possibility of creating something on my own terms.
A major milestone came when I hit the tipping point—I couldn’t keep up with both the clinic job and my growing private practice. Leaving a stable paycheck was definitely scary, but I knew I was building something that aligned more deeply with my values as a movement professional.
Since then, I’ve branded my business, created systems to support sustainable growth, and continued to refine how I deliver care. Now I get to help people move better, feel stronger, and stay active doing what they love—all while having the freedom to do it in a way that feels meaningful and aligned. Every risk has absolutely been worth it.
Where do you think you get most of your clients from?
The best source of new clients has consistently been word-of-mouth referrals. Because my approach is highly individualized and results-driven, clients naturally share their experiences with others who are dealing with similar issues or looking for a more holistic, movement-based approach to rehab and fitness.
What’s been especially powerful is the wide range of professionals who refer to me—doctors, personal trainers, Pilates instructors, chiropractors, and even other physical therapists. I think the common thread is that they trust I’ll take great care of their clients or patients and keep them moving safely and effectively. Building those relationships over time has been a huge part of my growth, and I’m incredibly grateful for the support of this extended professional network.
